I have avid 10 brakes with the 1.9 levers, and just the other day after i was done riding i noticed that the part of the cable that was exposed over the top of the frame of my bike was very loose, and i could pull it and activate the brakes with ease. So I looked up at the lever an noticed that the metal cap to my rear brake was only halfway into the lever. I tried adjusting it in all ways, but i can't get the metal piece on the end of the cable cover to go into the lever. Does anyone know what's up?

(And the cable is in the lever, so the brakes work)

I suggest you take it to your LBS. I, for one, am not able to follow your description of what is wrong. Perhaps if you could show someone what is wrong as you describe it to them...
